Remote Care Monitoring Program
ABOUT THE PROGRAM

A dedicated Remote Care Monitoring Coordinator through the Canadian Mental Health Association Fort Frances Branch provides follow-up and connection to appropriate community-based programs to help address recovery needs and, if necessary, the social determinants of health.
