Remote Care Monitoring Program
ABOUT THE PROGRAM
The Remote Care Monitoring Program is an app-based program connecting clients assessed by a Nurse Practitioner as having mild to moderate anxiety and/or depression. Through the SeamlessMD app, clients and their healthcare provider can track their recovery progress through the completion of daily, weekly, and bi-weekly questionnaires. A dedicated Remote Care Monitoring Coordinator through the Canadian Mental Health Association Fort Frances Branch provides follow-up and connection to appropriate community-based programs to help address recovery needs and, if necessary, the social determinants of health.
WHO IS ELIGIBLE
Patients 18 years of age or older assessed by a Nurse Practitioner of the Fort Frances Family Health Team as having mild to moderate anxiety and/or depression according to the GAD-7 and PHQ-9 assessment tools.

- Safety Planning
- Referral and Advocacy
HOW REFERRALS ARE MADE
Referrals are made by Fort Frances Family Health Team Nurse Practitioners to the Remote Care Monitoring Specialist.
Devices available to borrow by patients enrolled in the program.
