Case Management Program

About the Program
The Case Management Program is designed to facilitate recovery and maintain a quality of life directed by the individual.

The Community Support Workers will develop an individualized service plan to assist persons in developing the skills and supports necessary to maintain a level of independence that allows them to live and thrive in the community. Supports are provided in the community setting or within the person’s home environment according to the individual’s choice.

Who is Eligible
This program is designed to assist and support adults 18 years of age and older who have a serious mental illness.

The Program Provides

Case Management Services can be brief or long-term support.

The Program is Offered
Within the Fort Frances Area

How are Referrals made?
The CMHA Community Support program accepts referrals initiated by the family doctor, psychiatrist, community agencies, family members, and self-referrals. Clients must voluntarily agree to services.
